Everyday LIFE

PACE (Program of All-inclusive Care for the Elderly) is a unique model of managed care service delivery for the frail community-dwelling elderly, most of whom are dually eligible for Medicare and Medicaid benefits, and all of whom are assessed as being eligible for nursing home placement according to the standards established by their respective States. The PACE model of care includes as core services the provision of adult day health care and interdisciplinary team care management, through which access to and allocation of all health services is managed.

Services are provided in a PACE center, at a participant’s home, or in other settings where the delivery of care might occur. At the PACE center, the participant will receive vital socialization and recreation therapy, meals, rehabilitative therapy, social support services, and various medical services, including those of the primary care physician and nursing. The receipt and schedule of services is determined by each participant's plan of care.
